How the Public Health System Works in Cyprus: A Guide for Immigrants and Residents
Anyone who emigrates to Cyprus or lives permanently on the Mediterranean island sooner or later comes into contact with the state health system. Since the introduction of the General Health System (GeSY/GHS), medical care has changed fundamentally and today offers most residents access to modern and comparatively inexpensive health care.
Just a few years ago, Cyprus was one of the EU countries with the highest private health spending. Many patients had to finance doctor's visits and treatments themselves or take out private insurance. With the introduction of the GeSY, a uniform system was created that unites public and private service providers under one roof.
The General Healthcare
System (GeSY) is the statutory health insurance system of the Republic of Cyprus and is based on the principle of social solidarity: everyone pays according to their financial means, but receives medical services exclusively according to their actual health needs.
The system is financed by mandatory social security contributions, state subsidies and contributions from employers and the self-employed. Both state hospitals and numerous private doctors, clinics and pharmacies work together within the same network.
The family doctor as the first point of contact
A central component of the system is the so-called Personal Doctor (PD), comparable to the German family doctor.
Each adult insured person chooses his or her own personal doctor. Children are cared for by a registered pediatrician.
The family doctor takes care of:
- General examinations and preventive care
- Initial diagnoses and treatments
- Monitoring of chronic diseases
- Issuing referrals to specialists
In most cases, anyone who wants to use a specialist under the GeSY first needs an electronic referral from their family doctor. Without this referral, a specialist can be consulted directly, but the treatment is then carried out as a private service and must be paid for in full.
Especially in the case of longer therapies, for example after bone fractures or operations, patients can save considerable costs by making an official referral.
Electronic prescriptions and digital administration
The Cypriot healthcare system relies heavily on digitalisation. All prescriptions are recorded electronically and stored centrally.
Patients can then pick up their prescribed medication at any pharmacy affiliated with GeSY throughout the island. Paper prescriptions are largely a thing of the past.
How do I register?
Registration in the GeSY does not take place automatically upon entry into Cyprus. Those who live permanently on the island must register independently.

The process consists of several steps:
1. Create an online account
A personal user account is first created via the official GeSY portal.
2. Proof of residence permit
Among other things, you will need:
- Cypriot identity card or
- ARC number (Alien Registration Certificate)
- Proof of residence in Cyprus
EU citizens usually use their MEU1 registration, which is still known to many as "yellow slip".
3. Select a family doctor
After a successful examination, a personal doctor can be selected from the respective region of residence. As soon as the doctor confirms the admission, the registration is complete.
Contributions and costs at a glance
The GeSY is financed by mandatory contributions to income. The assessment basis is limited to an annual income of a maximum of 180,000 euros.

Low co-payments
In addition, small co-payments are made to protect the system from unnecessary visits to the doctor.
The most important fees:
- Family doctor visits: generally free of charge within the provided annual quotas Specialist

- Medication: 1 euro per prescribed preparation
- Laboratory tests: 1 euro per test, with maximum limits Emergency
- room: 10 euros
The annual cost cap is particularly family-friendly: the total co-payments may not exceed 150 euros per year. For children, low-income pensioners and other particularly vulnerable groups, this limit is only 75 euros per year.

Differences between EU citizens and third-country
nationals Access is comparatively straightforward for EU citizens. The prerequisite is legal residence in Cyprus as well as employment, self-employment or permanent residence status.
The situation is different for citizens from non-EU countries. They can only participate in GeSY under certain conditions, for example if they
- have:a permanent residence permit,work
- as an employee with appropriate social security rights,or
- have protection status as a refugee or beneficiary of subsidiary protection.
Tourists and people with short-term visit visas are generally excluded and need private health insurance.
International students
Different regulations also apply to students:
Non-EU students must take out private health insurance for their visa and cannot participate in GeSY.
EU students, on the other hand, can use their European Health Insurance Card (EHIC). However, as soon as permanent residence or employment in Cyprus is established, the switch to the regular GeSY system takes place.
Medical care for asylum seekers
Although asylum seekers do not participate in GeSY, they continue to receive medical services through the state health facilities.
The care includes, among other things:

- Treatment of acute
- illnesses Necessary medical care in public hospitals
For this purpose, a special health card is issued by the Ministry of Health, which is valid for one year at a time.

With the introduction of the GeSY, Cyprus has implemented one of the largest socio-political reform steps in its recent history. Today, both locals and permanent expats benefit from a system that connects public and private healthcare providers while allowing for comparatively low out-of-pocket costs.
